Under Florida law, only the minor's natural parent or legal guardian may provide consent.
Anyone under the age of 16 must be accompanied by their parent or legal guardian.
Minors 16 or older do not have to have their parent/legal guardian on site but still need to provide all the required documentation. Acceptable photo identification, proof of age, and proof of relationship include the following:
Photo ID/Proof of Age:

Valid, non-expired driver's license

Passport

State/government issued identification card (excluding Military identification) OR

A public school-issued photo ID or yearbook (with photo and name, accompanied by a birth certificate)
​Proof of Guardianship:

Original or certified copies of birth certificate, custody paperwork, adoption paperwork or guardianship paperwork
Parental Notarized Consent Form:

Available to download and print, as well as in the studio

Notary services are offered on-site for a $5 fee.

Minor Lobe Piercings for ages 7-11
(we do not offer piercings to children under the age of 7)

A consultation is required first. If the piercer is comfortable moving forward with piercing your child, they will schedule the piercing appointment at the end of the consultation appointment. No exceptions

All minor documentation is required at the time of the consultation

Your child must be mature enough to take care of the piercings themself and be provide verbal & written consent

If your child does not do well with shots, can't sit still for long periods of time, plans to swim in the next 6 months and/or plays contact sports, please consider waiting
